What to Expect at Your Visit
During your appointment, Dr. Mitchell will review your medical history, current health concerns, medications and supplements, lifestyle factors, nutrition, stress management, and personal health goals. A comprehensive physical examination will be performed, and additional testing may be ordered if needed.
At the end of your visit, Dr. Mitchell will discuss personalized treatment recommendations and next steps based on your individual needs.
Please arrive 15 minutes early for your appointment and complete your patient forms in advance whenever possible. If you have additional medical records or information related to your health history or current treatments, please bring them with you or submit them to our office as directed.

Medical History Forms
Please complete your medical history form with accurate and up-to-date information about your health, medications, allergies, and previous medical care. This information helps our rheumatology team better understand your health and prepare for your visit.
RAPID3 Patient Assessment
Please complete the RAPID3 questionnaire to help our rheumatology team assess your symptoms, physical function, and overall health. This will allow us to be able to track your health overtime and measure improvement in you health.
Insurance Card Upload Form
Please upload clear, legible images of the front and back of your current insurance card. Make sure all information is visible, including your name, member ID, group number, and any other insurance details. If you have more than one insurance plan, please upload the front and back of each card.
Medical Records Release Authorization
Please complete this form to authorize AZ Autoimmune Arthritis & Rheumatology to request or release your medical records from another healthcare provider. This helps us obtain the information needed to provide coordinated and informed care.

Costs, Insurance & Financial Policies
Cost of Medical Services
Initial Consultation
$450 per visit (60-90 Minutes);
available in-office or electronically.
Follow-Up Consultation
$350 per visit (45 minutes);
available in-office, electronically, or by phone.
Cancellation & No-Show Policy
24-hour notice is required for appointment cancellations or rescheduling.
If an appointment is canceled or rescheduled with less than 24 hours’ notice, or if you do not show for your scheduled appointment, you may be charged 50% of the appointment fee, unless there is an acceptable reason determined by the practice.
We understand that unexpected circumstances can arise. Please contact our office as soon as possible if you are unable to keep your appointment.
Billing & Insurance Information
In Arizona, Naturopathic Medical Doctors are not covered by insurance at the time of service; however, we accept Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA) for treatment services and supplement purchases.
We process insurance for in-house lab work and can provide lab orders for blood draws at outside laboratories such as Sonora Quest or LabCorp if needed.
Please contact our office with any questions regarding pricing or insurance coverage. All visits at AZ Autoimmune Arthritis and Rheumatology are self-pay. At this time, we do not contract with or bill medical insurance for office visits. Payment is due at the time of service.
